What to expect at your first visit:

You will meet with 2 people for your first visit: a staff member and Dr Shah. Dr Shah will review your pertinent history and help understand the root causes of weight gain and/or difficulty with weight loss. A second staff member will review your insurance benefits for weight loss counseling and review the overall treatment plan.

If you have had any labs done in the last year, please bring them to your first appointment. Ideally these are labs done by a primary care doctor and include the following labs: basic metabolic panel, liver function tests, TSH, lipid panel, fasting glucose test or Hba1c, and uric acid test (only if you have a history of gout).

If you have not seen a primary care doctor or don’t have these labs with you, Dr Shah will order them for you at the first visit.

If you are unable to complete this form prior to your appointment, please arrive 30 mins before your scheduled appointment to fill it out in the clinic.
